This panel measures the hormones most relevant to this transition, including the pituitary signals that rise as ovarian function declines, the oestrogen and progesterone levels that fluctuate unpredictably in perimenopause, and thyroid function — which commonly changes during this period and produces symptoms almost identical to hormonal menopause. Whether you’re in your late 30s noticing early changes or well into the transition, this panel gives you a clear hormonal picture to bring to any conversation with a practitioner.

What we test for

FSH (Follicle Stimulating Hormone)
The single most diagnostic marker for menopausal transition. As ovarian function declines, the pituitary gland produces more FSH in an attempt to stimulate the ovaries. A persistently elevated FSH is the clearest hormonal indicator that perimenopause or menopause is underway.

LH (Luteinising Hormone)
Works alongside FSH to regulate the menstrual cycle and ovulation. Rises during the menopausal transition alongside FSH. The FSH/LH ratio provides additional context around where a woman sits in the transition.

Oestradiol (E2)
The primary form of oestrogen produced by the ovaries. Fluctuates unpredictably in perimenopause before declining in menopause. Low oestradiol is associated with hot flushes, vaginal dryness, poor sleep, mood instability and accelerated bone loss.

Progesterone
Often the first hormone to decline in perimenopause, before oestrogen drops significantly. Low progesterone causes irregular cycles, sleep disruption, anxiety and heavy periods — symptoms frequently attributed to stress rather than hormonal change.

AMH (Anti-Müllerian Hormone)
A direct marker of ovarian reserve — how many eggs remain. Declines steadily from the mid-30s and reaches very low levels in perimenopause. Useful for understanding where a woman sits in her reproductive timeline and how close to menopause she may be.

Testosterone (Total & Free)
Often overlooked in women’s hormonal health, testosterone plays an important role in libido, energy, mood, muscle mass and cognitive clarity. Declines gradually from the 30s onwards and falls further during the menopausal transition.

SHBG (Sex Hormone Binding Globulin)
A protein that binds to sex hormones and affects how much is biologically active. Elevated SHBG can reduce the amount of free testosterone and oestrogen available to tissues, worsening hormonal symptoms even when total hormone levels appear adequate.

DHEAS
A precursor hormone produced by the adrenal glands that converts to oestrogen and testosterone in peripheral tissues. Declines with age and plays a supporting role in energy, mood, immune function and hormonal balance — particularly relevant as ovarian production declines.

TSH, Free T3 & Free T4
Thyroid dysfunction is common in women over 40 and produces symptoms — fatigue, weight gain, mood changes, brain fog, irregular periods — that are nearly identical to perimenopause. Testing thyroid function alongside reproductive hormones is essential to distinguish between the two or identify when both are contributing.

Cortisol
Chronic stress and elevated cortisol directly suppress oestrogen and progesterone production and worsen sleep disruption, weight gain and mood instability during the menopausal transition. Cortisol also rises when oestrogen falls, creating a feedback loop that amplifies symptoms.
