ANA (Antinuclear Antibodies)
The primary screening test for autoimmune disease. ANA detects antibodies that attack the body’s own cell nuclei — a hallmark of systemic autoimmune conditions including lupus, Sjögren’s syndrome, mixed connective tissue disease and others. A positive ANA result doesn’t confirm a specific diagnosis but is the essential first step in autoimmune investigation.
Anti-dsDNA
A more specific antibody test used when ANA returns positive. Anti-dsDNA is strongly associated with systemic lupus erythematosus (SLE) and tends to correlate with disease activity. Elevated levels alongside a positive ANA significantly increase the likelihood of a lupus diagnosis.
Rheumatoid Factor (RF)
An antibody associated with rheumatoid arthritis and several other autoimmune and inflammatory conditions. Elevated RF is found in around 80% of people with rheumatoid arthritis but can also appear in Sjögren’s syndrome, certain infections and other systemic conditions.
Anti-CCP (Cyclic Citrullinated Peptide)
A more specific and sensitive marker for rheumatoid arthritis than RF alone. Anti-CCP can be elevated years before joint symptoms develop, making it a valuable early indicator. A positive anti-CCP alongside RF is highly specific for rheumatoid arthritis.
ESR (Erythrocyte Sedimentation Rate)
A general marker of systemic inflammation. Elevated ESR indicates that significant inflammation is present somewhere in the body and is commonly raised in autoimmune conditions, infections and inflammatory diseases. Used alongside more specific markers to build a complete picture.
High-Sensitivity CRP (hsCRP)
A sensitive marker of acute and chronic inflammation. Rises quickly in response to immune activation and is commonly elevated during autoimmune flares. Useful for tracking inflammatory burden and monitoring how active a condition is over time.
Complement C3 & C4
Proteins that form part of the immune system’s complement pathway. In active autoimmune disease — particularly lupus — complement levels fall as they are consumed by immune complex formation. Low C3 and C4 alongside positive ANA and anti-dsDNA is a significant finding that warrants urgent specialist review.
TPO Antibodies (Thyroid Peroxidase)
The primary marker for autoimmune thyroid disease — Hashimoto’s thyroiditis and Graves’ disease. Autoimmune thyroid conditions are among the most common autoimmune diseases in Australia, particularly in women, and frequently present alongside other autoimmune conditions. Often causes fatigue, weight changes and mood symptoms that are attributed to other causes for years before diagnosis.
